QuadMed is seeking a Full-Time RN Lead to join our HII Family Health Center in Newport News, VA!
Schedule Options: Monday to Friday 8:00AM - 5:00PM or Tuesday to Friday 7:00 AM - 6:00PM. Off every weekend and paid holidays off!
GENERAL PURPOSE OF JOB
The Registered Nurse Lead supports the Medical Director, clinic providers, and staff in implementing and maintaining evidence-based patient care practices. This role focuses on increasing the delivery of preventive services and strengthening communication and collaboration among patients, providers, and the care team.
The Registered Nurse Lead provides general nursing care and leadership within a designated health and wellness center (or centers). Working closely with physicians and interdisciplinary team members, this role is actively involved in the daily care of clinic patients. Key responsibilities include, but are not limited to: collecting and documenting assessment data, recording vital signs and chief complaints, identifying allergies and relevant health history (with attention to environmental and discharge planning needs), administering medications, immunizations, injections, and therapeutic treatments safely and accurately, conducting patient triage, and delivering patient education—with a strong emphasis on wellness and prevention.
KEY RESPONSIBILITIES
• Deliver nursing care in alignment with ANA Nursing Standards, the Nursing Code of Ethics, and evidence-based primary care practices, while following universal precautions and infection control protocols. Maintain patient confidentiality in compliance with HIPAA and uphold patient rights at all times.
• Assess, plan, coordinate, and evaluate nursing care based on each patient's age, physical condition, psychosocial factors, and cultural background, following QuadMed standards and client requirements.
• Serve as a patient advocate by providing case management, education, and support to patients and their families.
• Provide basic wellness and prevention education that promotes self-care and is age-appropriate and tailored to individual learning needs.
• Prepare examination rooms with necessary supplies and equipment; assist providers during exams and procedures as needed.
• Support clinic goals through efficient, patient-centered care that is timely, evidence-based, and well-coordinated.
• Accurately document care and services in the Electronic Medical Record in a timely and compliant manner.
• Participate in quality improvement initiatives, continuing education, and patient care programs as required by QuadMed and client organizations.
• Assess and provide feedback on clinical, operational, and organizational practices to support improvements; use practice-level data to identify opportunities for change.
• Train staff in quality improvement (QI) principles and practices; facilitate clinical quality teams and manage related projects and changes.
• Oversee clinic workflows and operational effectiveness; enhance team capacity in using technology to improve clinical care and efficiency.
• Maintain required competencies and complete all mandatory training within designated timelines.
JOB REQUIREMENTS
Education:
- Graduated from an accredited nursing program, BSN preferred
Experience:
• Minimum of five (5) years of experience in a clinical setting, with strong proficiency in using electronic medical record (EMR) systems.
• Demonstrated proficiency in specimen handling.
• IV insertion and therapy skills preferred, with the ability to manage IV-related procedures and provide appropriate patient care.
• Prior leadership or charge nurse experience, with a proven ability to guide clinical teams, manage workflows, and foster a collaborative care environment.
• Experience in outpatient care and working with pediatric populations is strongly preferred.
Certificates, Licenses, Registrations:
- Current and valid nursing license in the state of the position or compact state license required
- Current CPR (BLS) certification or must be obtained within 60 days of hire
- PALS a plus
Knowledge, Skills & Abilities:
• Strong clinical nursing skills, with the ability to assess, plan, and deliver patient-centered care using evidence-based practices.
• Effective verbal and written communication skills, including the ability to provide patient education and contribute to clinical documentation.
• Strong organizational and time management skills, with the ability to prioritize tasks and work independently in a fast-paced environment.
• Critical thinking and problem-solving skills with a high level of attention to detail.
• Bilingual a plus.
• Proven ability to work successfully with diverse populations and demonstrated commitment to promote diversity and inclusion.
• Proficient in using electronic medical record (EMR) systems and related clinical technologies.
• Demonstrated ability to lead and support clinical teams, manage workflows, and facilitate collaboration across disciplines.
• Familiarity with quality improvement methodologies and ability to apply QI tools and strategies in a clinical setting.
• Commitment to continuous learning and professional development.
• Excellent interpersonal skills, with the ability to engage and build trust with patients, families, and care team members from diverse backgrounds.
